New Patient Evaluation
Health Assessment for New Patients
Service Description
What Is the New Patient Evaluation? The new patient evaluation is more than just a first visit. It is the starting point of a relationship. At IWC Primary Care, this visit is designed to feel thoughtful and unhurried. It begins with listening. Time is set aside to understand not only your medical history, but how you have been feeling, what concerns you may have, and what you are hoping to improve moving forward. Many patients come in unsure of what to expect or accustomed to shorter visits. This experience is meant to feel different. Our clinic visits are the preferred option for new patient appointments because they allow for a more complete and personal connection. At the same time, we understand that certain situations may make it difficult to come into the office. When needed, remote options are available for patients who are traveling from a distance, managing mobility challenges, or dealing with a contagious illness. This visit typically lasts 30 to 45 minutes. During that time, the provider will ask detailed questions to better understand your medical history, family history, current conditions, medications, and any symptoms you may be experiencing. Just as important, there is time for you to ask questions and share what matters most to you. The goal of the evaluation is not simply to gather information. It is to create a clear starting point and begin building a plan that feels realistic and tailored to your life. Together, you and your provider will establish personalized health goals and outline next steps that support your long-term health. Who Should Book a New Patient Evaluation? Anyone who has not previously been seen at IWC Primary Care should begin with a new patient evaluation. This visit is especially helpful for those who are looking for a primary care team that takes time to listen, explain, and follow through. Whether you are managing ongoing conditions, seeking preventive care, or simply looking for a fresh start with a new provider, this visit provides the foundation. What Should I Bring to the Evaluation? Bringing a few key items can help make the visit as productive as possible. Patients are encouraged to bring a list of current medications with dosages, any recent lab results or medical records, and information about past procedures or surgeries if available. If these are not readily accessible, that is okay. The care team will work with you to gather what is needed over time.
