Service Description

The Medicare Annual Wellness Exam is not rushed and it is not just a checklist. It begins with listening. Not interrupting, not moving too quickly, but taking the time to understand what has been going on and what matters most. Many patients come in unsure of what to expect from Medicare or feel like their visits have been too brief in the past. By the end of this visit, the goal is for things to feel clearer, more organized, and easier to manage with a plan that truly fits your life. This appointment creates space to talk through your full medical history, your current concerns, and how you have been feeling day to day. Medications are reviewed carefully to make sure each one still serves a purpose and is not causing unnecessary side effects. Preventive care is approached in a thoughtful way so it feels practical, not overwhelming. Lab work is explained in plain terms so you understand what the results mean and how they relate to your overall health. When appropriate, additional or more in-depth lab analysis can be coordinated to help identify patterns that may have been overlooked over time. The focus is not only on managing symptoms, but on understanding what may be contributing underneath. Whether it is fatigue, weight changes, blood sugar concerns, sleep issues, or ongoing discomfort, this visit allows the care team to step back and look at the full picture. From there, a plan is created that feels reasonable, sustainable, and tailored to your needs and priorities. If specialty care is needed, the process does not stop with a referral being placed. The reason for the referral is explained clearly, the right specialist is identified, and the team helps ensure the appointment is scheduled and completed. Follow through matters, especially with Medicare Advantage, and patients are supported every step of the way. We accept most Medicare Advantage plans including SCAN, Anthem Blue Cross, Brand New Day, Molina, Aetna, Alignment, Humana, UnitedHealthcare, Health Net, Blue Shield of California, Clever Care, Astiva, and Sharp Health Plan. Medicare Part B and Medicare Supplement plans, including TRICARE for Life, are also accepted. Many patients say the same thing after their first visit. That someone finally listened. That their concerns were taken seriously. That things were explained in a way that made sense. That the experience felt different from what they were used to. A better Medicare experience can start with one thoughtful conversation.
